Meropenem
Indications/Uses
Intra-abdominal infections, gynaecological infections, meningitis, resp tract infections (including in cystic fibrosis patients), septicaemia, skin & structure infections, UTI & infections in immunocompromised patients.
Dosage/Direction for Use
Adult Pneumonia, UTI, gynaecological infections, skin & skin structure infections 500 mg IV every 8 hr. Nosocomial pneumonia, peritonitis, presumed infections in febrile neutropenic patient, septicaemia 1 g every 8 hr. Meningitis 2 g every 8 hr. Childn >50 kg Adult dose, >3 mth up to 12 yr 10-20 mg/kg IV every 8 hr. Meningitis 40 mg/kg IV every 8 hr. Febrile episodes in neutropenic patient 20 mg/kg every 8 hr. Renal insufficiency CrCl 26-50 mL/min Recommended dose every 12 hr, 10-25 mL/min ½ recommended dose every 12 hr, <10 mL/min ½ recommended dose every 24 hr.
Special Precautions
Critically ill patients w/ known or suspected Pseudomonas aeruginosa lower resp tract infection (as monotherapy). Previous hypersensitivity reaction to β-lactams. Discontinue use if an allergic reaction occurs. Overgrowth of nonsusceptible organisms w/ prolonged use. Pseudomembranous colitis. Pregnancy & lactation.
Adverse Reactions
Pains, inflammation, thrombophlebitis at the site of inj; urticaria, rash, pruritus, erythema multiforme, toxic epidermal necrolysis & Stevens-Johnson syndrome; nausea, vomiting, abdominal pain, diarrhoea, pseudomembranous colitis; increased eosinophils, reversible thrombocythaemia, thrombocytopenia, leucopenia, neutropenia, agranulocytosis, +ve direct or indirect Coombs' test, shortened partial thromboplastin; increased transaminases, alkaline phosphatase, lactate dehydrogenase & bilirubin; paraesthesia, headache, seizures; rarely, systemic allergic reaction (hypersensitivity), angioedema & anaphylaxis; oral & vag candidosis.
Drug Interactions
Inhibited renal excretion resulting to increased plasma conc & prolonged elimination t½ w/ probenecid. Reduced serum levels of valproic acid to subtherapeutic levels.
